Welcome to the UH Elyria Medical Center Follow My Health patient portal..  Accessible from any computer, tablet or smart phone, Follow My Health allows you to receive lab results; email your doctor; pay bills online; and so much more - all from the comfort of your own home, or on-the-go-available 24/7!

What is it?

Follow My Health is an online tool that provides anywhere, anytime access to your personal health records and enables you to take a proactive role in managing your care.  This free self-service health management tool can improve interactions with your doctor's office, document important health care information and ultimately give you the ability to make more informed decisions about your health!

Why should I use it?

With Follow My Health, you can:

• Review your medical records online in a safe, secure environment
• Communicate privately with physicians via secure messaging
• View test and lab results
• Update your health information (allergies, medications, conditions, etc)
• Request RX refills
• Request or change appointments
• Fill out and submit forms prior to appointments
• View and pay bills

And it's available online 24 hours a day, 7 days a week via any computer, mobile device or smart phone!

How Do I Sign Up for Follow My Health with UH Elyria Medical Center?

• You may sign up to use Follow My Health during an office visit.

  • The staff will verify your identity (photo ID required) and notify the Follow My Health team that you've signed up.

• Within 1-3 business days, you will receive an invitation via email to activate your Follow My Health account.

• The email invitation will include a link to the Follow My Health login page. From this page, you can login to Follow My Health using an existing Windows LiveID, Google, Facebook, or Yahoo account. If you do not already have one of these accounts, a link is provided to create one or you can use the secure Follow My Health login.

• You will be required to enter an INVITATION CODE, which is your four (4) digit year of birth. Please note: the invitation code is only active for 60 days.
